By the late 450s, Hephthalite expansion into Bactria/Tokharistan brought sustained pressure onto Sasanian frontier systems in Khorasan and along the Oxus, setting the stage for repeated Sasanian campaigns and tribute diplomacy.
Peroz I’s first major eastern campaign ended in ambush and capture; his release required concessions and signaled that the Hephthalites could dictate terms to the Sasanian crown in the east.
A subsequent failed campaign led to another Sasanian humiliation: Peroz was again captured and ransomed, and his son Kavad was left with the Hephthalites as a hostage—deepening Sasanian dependence on eastern diplomacy and payments.
Peroz launched a third campaign and suffered a disastrous defeat in 484 near Balkh; he was killed and much of the Sasanian army was wiped out, enabling Hephthalite dominance over key eastern cities (e.g., parts of Khorasan).
In the immediate aftermath of 484, the Karenid noble Sukhra mobilized forces and campaigned against the Hephthalites, compelling a settlement that recovered captives/treasure and stabilized the frontier enough for a political reset at court.
After Peroz’s death, Balash (r. 484–488) was elevated as king while eastern territories remained under Hephthalite pressure; effective power increasingly shifted to leading nobles who managed war, tribute, and succession politics.
In 488, Balash was removed and Kavad I was installed; the transition reflected how eastern military crises and tribute burdens magnified aristocratic leverage over the monarchy’s eastern policy and overall governance.
Kavad moved against Sukhra—first exile, then execution (493)—reducing a kingmaker’s control but also destabilizing elite coalitions that had managed the empire after the 484 disaster, complicating coherent eastern policy.
In 496 Kavad was deposed and replaced by Jamasp; Kavad escaped imprisonment and fled east to the Hephthalites, showing how Hephthalite backing had become a decisive factor in Sasanian internal politics.
In 498/499, Kavad returned with Hephthalite military support and regained the throne. The restoration reinforced Hephthalite influence over Sasanian policy, including continued financial obligations that later shaped western war-making.
Kavad’s need for funds—linked in sources to debts/obligations arising from Hephthalite support—contributed to the outbreak of war with Byzantium (502–506), illustrating how eastern-policy costs could drive western conflict.
Under Khosrow I, the Sasanians allied with the Western Turks (Göktürks) and defeated the Hephthalites in a decisive battle commonly dated to the early 560s (often c. 563). The Hephthalite Empire fragmented; territory north of the Oxus fell to the Turks while the Sasanians consolidated influence south of the river.
