Virginia Mason formalized its approach as the Virginia Mason Production System (VMPS), adapting TPS concepts such as continuous improvement (kaizen) and “lean” methods to health care. VMPS was framed as a long-term management system focused on patient-first care, high quality, and organizational sustainability. This gave the transformation a shared language and a consistent method for redesigning work.
Virginia Mason introduced the Patient Safety Alert (PSA) system, modeled on Toyota’s “andon” concept, to make it normal and expected for staff to report hazards immediately. The design required staff to stop any activity that could harm a patient and trigger a rapid leadership response and investigation. This was a major cultural shift: safety concerns were treated as urgent system problems, not individual blame issues.
Virginia Mason organized specialized internal expertise to coach teams and standardize how improvement events were run, including evolving into a Kaizen Promotion Office (KPO). This mattered because it built repeatable capacity: staff could learn methods, plan events, and sustain changes rather than relying on one-time enthusiasm. The KPO also helped set expectations for leaders to support improvements on the front line.
Virginia Mason used Rapid Process Improvement Workshops (RPIWs)—short, intensive “kaizen week” events—to quickly redesign workflows with frontline staff. By March 2004, the organization had conducted 175 RPIWs, reporting measurable reductions in inventory, space, lead time, travel distance, and setup time. These early results helped build momentum and showed that the method could change daily operations, not just plans on paper.
Patient Mary L. McClinton died after a preventable medical error at Virginia Mason. The organization publicly disclosed the mistake, and leadership and the board elevated safety to a top organizational goal for multiple years. The event became a turning point that reinforced “zero harm” thinking and intensified the use of VMPS methods to prevent repeat errors.
Following the 2004 tragedy, Virginia Mason embedded safety learning into organizational routines and recognition, including the Mary L. McClinton Patient Safety Award. The award linked improvement work to patient outcomes and helped spread practices across units rather than keeping them local. This supported a culture where preventing harm was treated as everyone’s responsibility.
In a widely cited 2005 PSA event, a nurse escalated a concern when a physician did not follow protocol; leadership intervened and supported the nurse, including taking the physician offline after retaliation concerns were reported. The incident mattered because it demonstrated that “stop the line” would be enforced even in high-status conflicts. This helped staff trust the PSA process and increased reporting over time.
Virginia Mason applied VMPS concepts to redesign cancer care around patient flow—co-locating services and removing unnecessary delays. Reported early results included large reductions in chemotherapy preparation time, enabling the center to treat more patients with the same resources. The work illustrated how industrial-style flow ideas could translate into shorter waits and less patient burden in a clinical setting.
Virginia Mason created the Virginia Mason Institute to provide training and coaching to other organizations seeking to use VMPS methods. This step signaled that the organization viewed VMPS as a mature, teachable management system, not just an internal experiment. It also helped spread lean health care practices beyond Virginia Mason’s own facilities.
By 2010, Virginia Mason and outside coverage reported measurable safety performance improvements associated with VMPS, such as reduced inpatient falls compared with mid-2000s levels. Tracking these outcomes mattered because VMPS emphasized “standard work” (agreed best-known steps) and problem-solving at the source, which should show up in patient harm indicators. Documented performance trends helped justify continued investment through the end of the decade.
By 2010, Virginia Mason’s lean transformation was no longer a pilot phase: it had become an organization-wide management system with routine improvement events, a formal safety alert pathway, and dedicated internal support. The period from 2002–2010 shows a clear arc from adopting TPS ideas, to building infrastructure (PSA, RPIWs, KPO), to demonstrating sustained results and teaching others. This end state positioned VMPS as a lasting approach to patient safety and operational improvement rather than a short-term initiative.
