NASA planned mission AS-204 as the first crewed flight test of the Apollo Command and Service Module. The goal was to verify the spacecraft in Earth orbit before moving on to more ambitious missions. The crew named were Virgil “Gus” Grissom, Edward H. White II, and Roger B. Chaffee.
Command Module 012 (the AS-204 spacecraft) was taken out of an altitude chamber, where earlier tests had simulated conditions in space. Moving the vehicle into final launch preparations increased the pace of work and the number of changes being made. This set the stage for the “plugs-out” countdown rehearsal later in the month.
The spacecraft was attached to its Saturn IB launch vehicle at Launch Complex 34 at Cape Kennedy (now Cape Canaveral). This began a period of pad operations with repeated testing, installations, and troubleshooting. It also meant the upcoming rehearsal would occur in the full launch configuration.
On January 27, the crew entered the command module for a full countdown simulation with many systems running as if on launch day. During the test, the cabin was purged and pressurized with pure oxygen. Communications problems caused holds and added time to the test, extending exposure to the high-fire-risk environment.
A flash fire broke out inside the command module at about 6:31 p.m. EST. The pure-oxygen, higher-than-atmospheric-pressure cabin and extensive flammable materials allowed the fire to spread extremely fast and raise pressure. The inward-opening hatch could not be opened under the pressure, and all three astronauts died from asphyxiation caused by toxic smoke.
The day after the fire, NASA created the Apollo 204 Review Board to investigate the accident and recommend corrective actions. The board was directed by Langley Research Center Director Floyd L. Thompson and organized its work into many specialized panels. This started a formal, documented process aimed at finding causes and preventing recurrence.
Funerals were held four days after the accident. Grissom and Chaffee were buried at Arlington National Cemetery, while White was buried at the U.S. Military Academy at West Point. The ceremonies underscored the human cost of the program and increased pressure on NASA to fix safety weaknesses.
In February and March 1967, the Review Board’s panels reconstructed the spacecraft configuration and the test procedures used that day. They examined damaged hardware for evidence of electrical arcing and studied how the cockpit environment affected fire growth. A key conclusion was that the exact ignition source could not be uniquely identified, even though multiple arc sites were found.
The mission originally called Apollo 204 (AS-204) was redesignated Apollo 1 to honor Grissom, White, and Chaffee. This naming decision also shaped how later flights were numbered, with Apollo 4 beginning the sequence of subsequent missions. The redesignation tied the accident permanently to the program’s history and safety reforms.
The Review Board transmitted its final report to NASA Administrator James E. Webb, formally ending the board’s work. The report concluded the disaster resulted from a deadly combination: ignition likely from electrical wiring, a pure-oxygen high-pressure environment, extensive combustible materials, and an emergency exit that could not be opened quickly. It also recommended major changes to spacecraft design, test discipline, and quality control.
After the investigation, NASA and contractors redesigned the Command Module for fire safety. Changes included a unified hatch that opened outward, a major reduction in flammable cabin materials, and new approaches to cabin atmosphere during ground operations. These steps directly addressed the factors that had made the AS-204 fire so fast and unsurvivable.
Apollo 7 launched as the first successful crewed Apollo flight, showing that the redesigned spacecraft could operate safely in orbit. NASA explicitly applied Apollo 1 lessons, including an improved hatch and a safer prelaunch cabin atmosphere mixture during countdown. The mission restored confidence and enabled the later decision to attempt a crewed lunar-orbit mission with Apollo 8.
Launch Complex 34—site of the Apollo 1 fire and later Apollo 7’s launch—was ultimately left inactive and preserved as a memorial location. The surviving pad structures and plaques mark the accident’s place in Apollo history. As a physical endpoint, the site reflects the program’s shift toward stronger safety culture after the loss of the Apollo 1 crew.
